What most people don't realize but most of it is mind over body control, and meditation or yoga is done by most practitioners. While you need to maintain a level of concentration to open the lower sphincter muscle, most important is to have a level of calm, no fear and no panic - that can be fatal. There have been about 30 people killed doing this over the past 30-40 years, don't remember the exact dates.
