As I understand, that gesture is known (mostly in europe and the middle east) as "The Fig", and is right up there with flipping the bird as far as being offensive - I've heard of (but never with enough evidence to call it "iron-clad fact") cases where it's been classified the same as "fightin' words" - as in
Accused: Yeah, judge, you better believe I beat the everlovin' bejeezus outta him - he threw me the fig!
Judge: Oh... In that case, charges dismissed. Just don't beat anybody who doesn't. <gavel-bang>
 
Edwina Booth, while she still had a career....She went to Africa to shoot a film for Irving Thalberg, "Trader Horn".....And contracted several diseases, a serious issue with her skin because of a costume problem....sunburn at a very bad level...She fell from a tree and injured her back. Thalberg was asked to send doctors and ambulance facilities but did not, for fear of cost overruns. She sued after taking six years to recover from Malaria.
