Yeah I think that's the safest way to make it work. I wouldn't want to make the penis ring/sheath/covering too hard, part of the appeal of a knot I imagine is feeling a little bit of a squeeze down there, you just don't want it to be enough to cut the blood flow at all.

I think someone could easily market two versions of this. One that just has the necessities for the knot and leaves much of your cock bare, and one full penis sheath like the one in your picture so you have what looks like a dog cock plus it has an inflatable knot.

Just don't stick random plastics like that into any opening of any body, nonhuman or human, since many plastic compounds leech chemicals that are toxic. Medical grade silicone is safest from what I've read.
