Prostate treatment

I thought I'd share one of my boys journey with a common prostate issue as we attempt to treat him in the hope that someone else may find our experience interesting and or useful as I see the odd post here about canine prostate issues.

So, some background:
* He's 5 years old.
* Not a sexual partner, but he does ask for and enjoy the occasional handjob (maybe once a month? Sometimes more, often less). He has never been used at stud.
* About 88 pounds / 40kg.
* Fit, lean, energetic and (otherwise) healthy with no signs of straining to defecate or issues urinating. No other serious health concerns now or in the past.
* Lives with multiple large breed intact males but no females.

The problem:
A few months back I noticed some blood in his ejaculate after he'd had a wet dream (rare for him) while sleeping next to me. A collection confirmed that his ejaculate was a red (bloody) and a rectal exam revealed his prostate to be around double the size it was at his last check (I check it every year around his birthday). It was symmetrically enlarged, smooth with two apparent nodes and not painful. Blood in the ejaculate is a common symptom of an enlarged prostate (benign prostatic hypertrophy/hyperplasia or BPH).

It took a while to get an appointment with our vet but off we went fully expecting to come home with a booking for castration surgery but hoping for an alternative as he is a conformation show dog in addition to being my buddy and couch/bed hog. I was also concerned about any change in our pack dynamic that a neuter might cause as at the moment everything is quite stable and everyone gets along well enough...but I am still willing to go down that path if required as his general well being is more important than competitions.

We were given three options:
1: Surgical castration - almost certain (99.999%) to resolve this issue at the obvious expense of him being sterilized and no longer able to compete along with a possibility of upsetting the pack. The vet was actually hesitant to do this due to his age, living conditions, show career and as his prostate is not massively enlarged...yet.

2: Temporary chemical castration with a 6 month implant (Suprelorin) - this has a similar effect to surgical castration as it turns off the part of the brain that signals the testicles to make testosterone. His testicles would shrink to about half to one third of their normal size, he would be rendered infertile within a month or so after implantation and almost certainly experience anejaculation as his prostate would shrink to about 25% of its normal size. His show career would have to be on hold for the duration of this treatment due to the atrophy of his testicles (they are checked by every judge and must be apparently normal, baby balls are not going to fly!). Recommended to monitor him as the implant wore off and re-implant or surgically castrate if/when symptoms returned. Same concerns as option 1 regarding the pack dynamic but at least if everything went to shit it would wear off eventually, if all went well we could go to surgery.

3: Treatment with Finasteride - this should shrink his prostate by around 50% and have few or no other side effects except for a reduction in the volume of ejaculate, his balls stay normal and he remains fertile. It works by blocking the breakdown of testosterone that causes the prostatic enlargement. He may need to be on this for either a few months or possibly for life. Major concern with this was cost but as it turns out there are generic Finasteride tablets available for modest cost.

All three options came with a fistful of eyewateringly expensive antibiotics to treat any possible infection and a suggestion to wait at least a couple of weeks before starting any other treatment so as to allow the antibiotics time to do their thing while the prostate is still large and has lots of blood flow.

Which brings me to where we are now, he has been on antibiotics for a few weeks and a collection has revealed no change...if anything there was more blood. A quick rectal exam showed his prostate to be the same as it was prior to starting the antibiotics. So today we start option 3, the Finasteride. I'll post again in a couple of weeks with a progress report.
 
Sorry you're going through this with your boy, but it's really awesome of you to share such a detailed report.
I for one am interested to see how this turns out. This is probably one of the more common male dog issues when they get older, but I haven't come across anyone who tried Finasteride on their dog, and always wondered how dogs would do on it (health wise, social dynamic at home, libido).

Progress report.

It has been about three weeks since I started giving him Finasteride daily and it looks like it is starting to work. Yesterday he was asking for some attention so I gave him a collection which showed a completely clear first fraction, a nice cloudy second fraction and a very slightly red third fraction. That is a big change as his first fraction prior to treatment was so red it looked like watery blood. A rectal exam showed his prostate to be smaller but not as much as I'd hoped...maybe 10%? We are still quite early in the treatment though and from what I can gather it will be another 4 to 6 weeks before maximum effect is achieved.

Other things of note: there are no other things of note! He seems 100% normal in every way, appetite unchanged, libido unchanged (such as it was anyway), no difference around the other boys and no change in their treatment of him.

So far I'm really pleased with how this is going and am hoping I'm not jinxing things by saying that!
 
My boy lost some blood drops out of his sheat every few month and we didn't find out for the first what was the Issue. Even the vet couldn't tell us. After some month with no blooding dropds he also lost sime single blood drops out of his sheat. So we went again to the vet, did somd ultrasonic and a prostate investigation. And yeah there the vet found a bit bigger prostate and a bladder inflammetion. So we got some antibiotica for the bladder and somethink like Finasteride, so his prostate gets smaller. And after oe or two weeks, his prostate got normal again and also his bladder was fine again. And now - half a year later, my boy is quite fine.

@RTR: Maybe also check his bladder. Maybe your boy has also an inflammetion. This could cause a bigger prostate.

He was prescribed a range of antibiotics to take care of any possible infection, I actually waited three weeks (almost the entire course) after starting the antibiotics before giving him the first dose of Finasteride. The antibiotics did not appear to do anything, I do not believe there was any infection to treat.

I'm fairly certain my guys issue is 100% prostate as there was never any hint of blood in his urine nor did the other boys react to it as they have in the past when a visiting neutered dog had a urinary tract infection (they lick and chew the urine like its from a bitch in season, since then they are all obsessed with the urine of neutered males as well as females :sick:).

I glad you also managed to get your boy treated without having to resort to castration!
 
I have used finesteride with great success. I highly reccomend it in cases of enlarged prostate or in combo cases with prostitis and enlarged prostate. It does take a long time to get the antibiotics into the prostate to help with the prostitis and don't be surprised if you get flare ups every so often. Is way better than the alternative - I actually had a dog die on the table during a castration to treat this very issue which made me swear never again will I resort to surgery unless it is truly my only option. Just to be clear my dog did not die from surgery directly but from an undiagnosed heart condition that cause his heart to race and then stop when he was under anesthhia and they could not revive him. Surgery has very real risks and I advise anyone that is getting any kind of surgery to research alternatives.

There is an alternative treatment for enlarged prostate, called Pulsed Electromagnetic Field therapy (PEMF). It was originally designed for humans, but it's effective for dogs as well. It is non-invasive and can be done at home in just a few minutes per day.

I can't vouch for how effective it is, but it's something to consider. I have no idea what it costs.

I suggest reading research from leading medical institutions before putting trust in electro magnetic anything. I think it is misused to treat conditions it is not effective against. I talked to a tech for electro magnetic therapy and I asked what is it effective for? She listed out a dozen different conditions that affect all over the body.

I've read the information DingoJay is referencing, the article is based on a peer reviewed study published in The Prostate. It's been a while since I've felt compelled to experiment but the device used looks to be easy enough to copy, just some really strong magnets on a motorized turntable in a nice housing. It's likely that I'll have a crack at making something to see what effect it has as I've run into some difficulties obtaining Finasteride and my circumstances have changed significantly since I last posted - I'll share that story later.

I'm not saying I don't share the skepticism though.

I've read the information DingoJay is referencing, the article is based on a peer reviewed study published in The Prostate. It's been a while since I've felt compelled to experiment but the device used looks to be easy enough to copy, just some really strong magnets on a motorized turntable in a nice housing. It's likely that I'll have a crack at making something to see what effect it has as I've run into some difficulties obtaining Finasteride and my circumstances have changed significantly since I last posted - I'll share that story later.

Mind sharing the info you've found in the device? My curiosity here is based in the fact our boy had BPH and we visited a number of specialists and everyone said neuter, so after ulrasounds, urologists, and all, we did as advised. If there's an alternate treatment that works, it would be nice to know for future cases of BPH.

Also skeptical, but always open to changing my mind.

What did they say about finasteride?
Nothing? The paper is about PEMF and the only mention of finasteride I noticed is in the title of a reference:
McConnell JD, Roehrborn CG, Bautista OM, et al. The long-term effect of doxazosin, finasteride, and combination therapy on the clinical progression of benign prostatic hyperplasia. N Engl J Med.
2003;349:2387-2398.

Edit: the more I read the more skeptical I become.

I'm talking about the specialists when you decided to neuter your dog.
Ah, I misunderstood. Everyone was on the same page, neuter being the only option suggested despite us seeking out several specialists expressly for the purpose of finding an alternative. He was the first male dog I had into his senior years so it was a new experience for me, not having dealt with BPH myself at the time. Things worked out relatively well, his BPH was no longer a pressing issue, no pun intended, and it had only a partial impact on his sex drive...I know we got lucky there.

Ahh, that's extremely frustrating. I know for certain that the repro guys I can access will prescribe Finasteride for BPH, they've done so for some of my friends breeding dogs that have developed symptomatic BPH. Unfortunately for me, getting to those specialists is difficult just due to distance. I've attached a study here outlining the effectiveness of the treatment in canines.

I've attached a youtube video of it being demonstrated with some iron filings, you can see it uses a permanent magnet as the filings are attracted with the device turned off, activating it causes the filings to swirl following the rotating magnets underneath.

Well, I've been meaning to write this progress report for a while but things kept getting in the way...dead computers etc.

Late last year a new member joined our pack...for the first time in over a decade there's a female in our home. Coincident with her joining us the vet that was treating my boy experienced a serious downward spiral with his health meaning our supply of Finasteride dried up. Everything was still fine until late January when without medication symptoms inevitably returned. Now faced with a vet who wasn't as understanding of our circumstances and the prospect of the newly acquired bitch coming into season I made the decision to buy us some time and ordered a six month Suprelorin implant. The effects are pretty much as expected.

The good:
* His prostate is now about the size of a pea and he is completely asymptomatic, mostly because he can no longer ejaculate
* He is even snugglier than before, 150% cuddlebug
* There have been no issues with the other males in the household, if anything there's a bit less stress
* He's gained some body fat that makes him a lot nicer to have in bed, he's now kinda squishy? I'd call him pet weight now, while he still has a waist line and some tuck I certainly don't want him any heavier
* The extra body fat seems to keep him a bit more insulated, he doesn't seem to feel the cold as much
* His coat texture is much softer which again makes him much nicer to have in bed
* He will now eat chicken carcasses, his teeth are the best they've been in ages. Before it was a struggle to get him to chew anything much at all as he wasn't that interested in food

The neutral:
* His "stud butt" has abated...no more big puffy tail hole
* His conformation show career is on hold until his balls grow back
The bad:
* He's gained some body fat and lost a fair bit of muscle tone
* He now needs to be crated when I'm away from home or he scours the house for food, I used to be able to leave food on the kitchen counter and not worry about it being eaten. No more, every single crumb is now devoured if left unattended and random items are shredded in the search for anything edible
* Any weight gain is now a lot more difficult to shift
* His balls are now so small that he can easily be mistaken as being surgically castrated if the weather is even mildly cool
* The softer coat texture is incorrect for his breed
* He has reverted to some more puppy like behaviors
* He no longer keeps his penis as clean as before, it's completely dry and prone to getting stuck when he shows pink or after sex. He no longer shows much interest in licking it nor does he seem to care if its not put away properly. It's also lost that sweet dog dick smell and is actually a bit yuck now...it just smells like stale dog piss which isn't nice when he's cuddled up on a cold night

Sexually, I was collecting him once or twice a week following the implant so I could track the changes (I've no objection to jacking him off...for science ;)). There was little change after the first week, 14 days in and the volume of his ejaculate had dropped by two thirds. After three weeks it was down 90% and at the four week collection he was completely dry. Up to that point he'd maintained pretty normal sexual behavior, full erections lasting 15 to 20 minutes. Weeks 5 and 6 he was completely disinterested, no erection not even a bit of a hump of my hand. By eight weeks though his libido sprang back and he was back to fully humping and was able to get a partial erection, which was some of the strangest things I've seen...he'd have a fully engorged knot with the shaft still flacid for 10 minutes or more until he was finished :oops:. As time has gone by his penis has shrunk despite more regular attention, the erections while maybe 33% smaller are otherwise back to normal although more work is required on my part to get him up and keep him there. One benefit is at least we can have some fun and there's no "who has to sleep in the wet spot" issues so he actually gets more sex now than ever. On that subject, the girl has had her season and of course having confirmed the effectiveness of the implant they were allowed to run together, much humping and enjoyment was had by both of them and no puppies (phew!). We're now approaching 9 months since he was given the six month implant so it'll be wearing off soon which brings me to...

He's going to need surgery soon to remove some suspicious lumps and bumps and obviously the opportunity to castrate is there while he's under, at this point given how he's reacted to the implant I'm actually quite torn on the best way forward which isn't something I would have thought I'd say going into this. Quality of life wise he may actually be better off without balls, at least until the extra weight and muscle loss catches up with him. He's not stressed by being crated when I'm not around but there is a much higher chance of him eating something he shouldn't if I let my guard down and any food stolen turns immediately into fat. He wasn't stressed at all by having a girl in season here, none of his normal crying or going off his food and he actually got to use his dick for its intended purpose for a change. Even with that aside, he's getting more action than ever due to the lack of clean up noted above.

While I'm leaning toward letting things grow back as I belive I have just become used to some of the changes in him, I really don't know what the right way forward is...there doesn't seem to be an obviously "right" answer.
 
One of my males had the same issue blood in his ejacualtion. My vet was easily able to find his issue we did go a semen bacterial culture to rule out any type of infection. He does have BHP and is on meds to help. A month after he started his meds his ejaculations were free is blood. My vet did ultrasounds to before and after and confirmed his prostate was back to normal size.
