I don't think any of my turn ons surprise me at this point to be quite honest. I've got a lot of kinks and my fantasies go all over the place, they can range from pretty simple and sweet to "this is not possible in the real world". That has never really been something that has surprised me though. Surprise isn't the right feeling, it's more like confusion. I've definitely been confused as to why things turn me on because I didn't initially understand the appeal at first. I've got my fair share of violent/gorey fantasies, but I wouldn't call those surprising, nor is it confusing because I know what I like about it.

This is a big one for me too as a vore enjoyer. Maws, throats, honestly the internal view of various orifices in general. There's something so intimate about it to me.
